First, the accident passed

2006 at 23:20 on April 11, after a steel company in Liaoning Province iron end converter maintenance shutdown, plant equipment operating long lance conductor test, less than 2min time, after being ejected from approximately 1685m3 oxygen lance The suction flue is removed, and the drift is nearly 300m to reach the flue fan.

At 23:30, one of the fitter's clothes covering the flue fan was splashed with gas welding sparks, and the overall work clothes burned quickly. The workers working with the pliers immediately sprayed dry powder on the body with a fire extinguisher. After the fire was extinguished, it was taken out of the wind turbine and taken to the hospital. Due to extensive burns, the fitter died at 2:50 on the 12th.

Second, the cause analysis

1. The density of air and oxygen under standard conditions is 1.295g/L and 1.429g/L, respectively. Since the density of oxygen is slightly larger than the density of air, the oxygen cluster is not easily mixed with the atmosphere under the breeze meteorological conditions. After drifting 300 m along the ground, the fitter is surrounded by oxygen.

2. The work clothes of the working fitter in the oxygen group belong to the flammable gas. When the high-temperature gas welding spark is ignited, it is violently burned, and the fitter is severely burned to death.

Third, the accident lessons

1. Do not release large amounts of oxygen into the atmosphere at any location where there are multiple types of work.

2. In the place where there are many types of work, if there is a large amount of oxygen leakage accident, it is necessary to immediately notify the downstream workers to stop the operation within 1000m. It is best to evacuate the site and wait for the plant safety management personnel to check the oxygen using the oxygen detector. The operation can be resumed when the content reaches the normal value.

3. It must be operated under oxygen-rich conditions, and operators should not perform open fire operations such as electric welding, gas welding, and gas cutting. Not use sparking tools (ordinary steel wrenches, hammers, etc.), you should use copper alloy material is not sparking tools to prevent sparks caused by an explosion due to the tool.

4. Oxygen leaks into the atmosphere in large quantities. If there is a strong wind, the airflow is agitated sharply, the distance of the oxygen mass drifting along the ground is short, the risk of fire is small, and the possibility of burns by nearby people is small; if it encounters a breeze, The airflow diffusion speed is slow, and the distance of the oxygen mass drifting along the ground is long, which causes a high risk of fire, and the possibility of burns by nearby people is also large.
